broke Polsat (they use old-style MECM) in Conax CAM.

This is a maintenance release,
nothing major, just fixes for minor stuff I found out..

1. Fix in viaccess for Dreambox EMU's when they are starved for valid key-files.
   Now works with radegast's flawed ECM response request :()
     Why does it (radegast) ask for more data than it needs ??)
     "ca c0 00 00 1a" where a real CAM asks for "ca c0 00 00 12"..
     breaks most *p-cards* and EMU's..
     the "1a" is usually used as a flag to respond with issuer or other info,
     and only asked during startup :(
     A real viaccess cam don't behave like that :-()

   Now works with SCAM's version of a viaccess ECM.
     SCAM does not transmit the 9f geo-code nano as a separate nano ..
     also breaks most *p-cards* and EMU's..

2. Irdeto/Nagra idents are now uniquely identified
   example: 4801 is different from 4901
   so for idents that use the same key and differ only
   in ident info, you now need to provide multiple
   identical key/RSA-keys for those ident(s).

   Also found a bug :-()
    when receiveing an ident that is not in irdeto.dat it would hang indefinitely.

3. Added a way to import/use nagra RSA-keys in 'standard' format as used by for example newcamd.
   To use:
   Copy the key from rsakeylist..
   example: (1800:000000:00000000000000:710102:1_E1:CB7E9..)
   "CB7E9.." is the key to copy.
   prefix it with n_1, n_2, or e_1, and put it into irdeto.dat at the correct place,
   start WCE, press 'n' and WCE will write it back into irdeto.dat in it's own format.

4. Reset detect logic reworked (again <sigh>) it works better on my equipment,
   probably not on your's..but what do I care ;)

I know I forgot to mention things I did, memory being *teflon*(TM) slippery,
 and I'm also sure there are more things to fix/change... :()

If it works, great, if not...then just delete it ;-)

1. Seca2 ident 0065(Cyfra+) is open.
2. Card <-> CAM logging enhanced (just a little)
   Start WCE with command-line argument -lnn
   where nn is logg-level wanted.
   0: No card logging (Normal emulation mode)
   1: No card logging but some extra debug info printed.
   2: Normal logging.
   3: Log card ACK's too.
   4: Log all header data (logs ATR and all data).
   7: As 4 + Log card ACK's too.
3. Added timestamps on all input/output.
4. No, still no Sky Italy or D+ Spain.
5. Read 4. ;)

Spain ident 0064 D+ (Taquila etc) works again :)

Italy 0070 once again, it never stops ;)
